PARENT/GUARDIAN CONSENT FOR SCHOOL HEALTH SERVICES 
Under the Florida Parent's Bill of Rights and in accordance with Florida State Statute 1014.06(1), parents or legal guardians of students must authorize the school to provide limited healthcare services for your student, should the need arise for such treatment, while your student is under School supervision. Parents must also provide consent for students to participate in screenings required by Florida Administrative Code 64F provided at School, vision screening, hearing screening, growth and development screening, and scoliosis screenings. By responding "Yes" to this parental consent for healthcare, you are authorizing the school to provide limited treatment to your student for minor injury or illness reported or observed while your student is at school, including but not limited to, basic first-aid services, providing bandages and icepacks when applicable, checking temperatures and basic injury assessment. A "Yes" response does not authorize the dispensing of medication. The provision of medication requires a separate, additional consent. 
A "No" response will result in calls to the parent/guardian or emergency contact for all medical concerns including but not limited to instances where students may have a headache, feel ill or minor injuries such as cuts, scrapes, bumps, or bruises. In an emergency first aid, cardiopulmonary resuscitation (CPR) or use of an automated external defibrillator (AED) will be performed by employees until Emergency Medical Services (EMS) arrive without prior parent/guardian consent. EMS will continue to be contacted for any situation deemed serious. 
This consent will remain in effect until your child transfers to another school district, graduates or you indicate in writing that you wish to rescind this consent for school health services.

Failure to respond will result in an indication of “No” for healthcare treatment and a parent or guardian will need to be contacted for all medical concerns e.g., bandage, icepack etc.

We are pleased to inform you that Summerville Advantage Academy will be participating in a
FREE BREAKFAST AND LUNCH PROGRAM
for School Year 2025-26.  This alternative is referred to as the Community Eligibility Provision (CEP). Parent Letter  If your child attends Summerville Advantage Academy, breakfast and lunch will be available to them at no charge during the school year.  Household applications for free and reduced-price meals will not be collected.

By providing school meals for all children at no charge, we are hoping to create a better learning environment for our students.   Please encourage your children to participate in the school meal programs.

If you would like your student to have the opportunity to purchase additional breakfast or lunch items you will need to create an account to provide funds for ala carte purchases which will be available starting mid-September.

Our after school program offers a variety of themed events and activities throughout the school year as well as, outdoor sports, afternoon snack, and up to 45-minutes of dedicated time for students to complete homework. The program is fee based. Students may arrive at 6:30 a.m. for before school care and must be picked up by 6:30 p.m. from after school care.

Water Safety and Swimming Certification - Florida Statute 1003.225

Beginning with the 2022-2023 school year, each public school shall provide, to a parent who initially enrolls his or her child in the school, information on the important role water safety education courses and swimming lessons play in saving lives. The information must be provided electronically or in hard copy and must include local options for age-appropriate water safety courses and swimming lessons that result in a certificate indicating successful completion, including courses and lessons offered for free or at a reduced price. If the student is 18 years of age or older, or is under the age of 21 and is enrolling in adult education classes, the information must be provided to the student.
